Cyclosporiasis outbreaks expose the growing gaps in global food safety systems

The ongoing Cyclosporiasis crisis in the United States, the largest on record, is exposing a vulnerability that is at the heart of global food trade: food can cross borders within days, but detecting the contamination that makes thousands of consumers ill, tracing it back to its source and coordinating a response can take far longer.

More than 24,000 confirmed and probable infections have been reported across 47 U.S. states, with over 800 hospitalizations and two deaths. The Food and Drug Administration (FDA) has linked 10,930 cases in 17 states to shredded iceberg lettuce imported from Mexico and served at various store and restaurant chains in the USA.

What is Cyclosporiasis?

Cyclosporiasis is an intestinal infection caused by a tiny parasite called Cyclospora cayetanensis. People become infected by consuming food or water that has been contaminated with human feces.

The parasite needs one to two weeks in the environment to become infectious but the infection does not spread from person to person. Symptoms typically include watery diarrhea, bad stomach cramps, bloating, nausea, and extreme tiredness. Without treatment, the illness can last for weeks or even months. Most often, experts say, the difficulty is not the treatment, but establishing where people became infected in the first place.

A difficult parasite to trace

Scientists cannot easily track Cyclospora because it cannot be grown in a lab as most bacteria can. Instead, they must copy its DNA from samples, which is less precise and can cause false positives. Scott Gottlieb, a former Commissioner of the FDA, explained that there is no whole-genome sequencing for Cyclospora to create a genetic fingerprint and link an outbreak to a source.

Testing is also challenging. Cyclospora is not included in routine stool tests for patients with diarrhea, so therefore many mild cases may go unreported. The parasite can take up to 14 days to cause symptoms, which makes it difficult for patients to recall what they may have eaten when they initially became infected. As the parasite is often found in mixed foods such as salads, this makes the source even harder to identify.

Cyclospora is also difficult to control because washing does not kill it, specialists warn. Once crops have been contaminated, halting the spread is much more difficult.

The U.S. food safety system under pressure

The outbreak has unfolded amid significant cuts to U.S. public-health capacity. In 2025, the FDA lost about 20% of its staff, while the Centers for Disease Control and Prevention lost 27%. These cuts have also reduced the number of federal food inspections and affected the payment of grants to state and local health departments.

Dr. Susan Mayne, Professor of Epidemiology at the Yale School of Public Health, has highlighted that cuts to public health capacity have affected the teams responsible for communication, international coordination and emergency response.

The outbreak does not stop at the U.S. border

The Cyclospora problem is not confined to the U.S. alone. Although neighboring Canada has described the number of registered Cyclospora cases as “typical”, nevertheless Quebec reported an increase of over 30% in the number of cases as of July 2026 compared to the same period in 2025.

Europe has not reported any outbreaks, but has stepped up surveillance after sporadic cases tied to travel to Mexico, Peru, and Guatemala were registered. The Netherlands, which does not routinely track the infection, has reported 13 cases, some of them tied to travel to Mexico. The UK registered 67 cases between April 30 and July 15, 2026, mostly in travelers returning from Mexico.

None of these cases are considered to be extensions of the U.S. outbreak. They do, however, illustrate the difficulty of monitoring infections that originate in one country and are then detected in another.

One nation’s food crisis becomes a regional threat

Foodborne outbreaks are not simply a public health problem. Caused by a local incident, they can often become a national disaster and grow into multi-state and international health crises given the cross-border nature of trade. A failure at a farm or a processing facility can result in sickness in multiple countries unless robust surveillance is in place.

For public health authorities, this means that national surveillance systems must increasingly depend on information shared across borders. The United Nations has noted that “effective response depends on surveillance data, scientific collaboration and early-warning systems that must move rapidly across borders”.

The significance of the Cyclosporiasis outbreak lies not only in the number of cases, but also in how difficult it is to coordinate modern food safety systems when contamination moves through complex international food supply chains.

As U.S. Senator Amy Klobuchar has warned, recent U.S. policy changes “have significantly weakened federal and state defenses and undermined nationwide coordination against foodborne illnesses”, making it harder to report and compare data across state lines.

What needs to change

The lesson from the Cyclosporiasis crisis seems clear: food safety systems must keep pace with global food trade. That means more robust disease surveillance, better laboratory capacity, quicker information sharing among countries and more effective systems for tracing food from producers to consumers, as the World Health Organization highlights. When contamination crosses borders, surveillance data, laboratory findings and warnings must cross them just as quickly. The effectiveness of the global food safety system depends on whether those connections happen when they are most needed.
