Contract name: Medical equipment and medical products to equip ZOCCH (for the diagnosis and treatment of patients with ischemic heart disease and rhythm disorders).

Reference No.: ICB 3.1.10.

1. The Government of Ukraine has received a loan from the World Bank toward the cost of the "Improving health in the service of the people",  №8475-UA Project and intends to apply part of the proceeds toward payments under the contract for procurement of Medical equipment and medical products to equip ZOCCH(for the diagnosis and treatment of patients with ischemic heart disease and rhythm disorders) (package №3.1.10.). Bidding will be conducted according to the rules and procedures of the World Bank.

2. The Healthcare department of Zakarpattia Oblast State Administration now invites sealed bids from eligible bidders for the supply of the following goods:

Lot 1: «System of electrophysiology studies (126 channels)» – 1 piece.

Lot 2: «Cryoablation system» - 1 piece.

Lot 3: «Modernization and workability restoration of  angiography system Philips Allura Xper FD10» - 1 piece.

Lot 4: «Expert class ultrasound diagnostic apparatus    for examination of internal organs » – 1 piece.

Lot 5: «Mobile C-arch (mobile X-ray system with C-arm for heart and blood vessels examination)» – 1 piece.

Lot 6:

  • «System for remote monitoring with implantable cardiac monitors and defibrillators» – 1 piece.
  • «Cardiovascular operating table» – 1 piece.
  • «Holter monitoring system with registration of daily ECG(3-channel) with possibility of continuous recording and analysis of ECG during up to 7 days (10 recorders)» – 1 piece.
  • «Electric oscillation saw for sternum opening» – 1 piece.
  • «Electric fret-saw for sternum opening» – 1 piece.

Bidders shall meet the following qualification criteria:

(1) Presence of at least 5 years of experience in manufacturing of equipment. The qualification of bidders who submit bids for several lots will be evaluated on cumulative basis.

(2) For local Bidders evidence of not having deferred debts to Tax authorities (a certificate from the Tax authorities on the absence of arrears of taxes and obligatory payments).

(3) C onfirmation of the minimum average annual turnover equivalent to the proposed bid value calculated as total certified payments received for contracts in progress or completed, within the last three (3) years starting 1st of January, 2015 prior to the deadline for bid submission.

 (4) Successful completion of at least two (2) contracts similar to the offered cost of each:

The qualification of the Bidders submitting bids for several lots will be assessed separately for each lot.for lot 1 - must be equivalent or more than the equivalent of 200 000 USD during the last 3 years;

for lot 2 - must be equivalent or more than the equivalent of 200 000 USD during the last 3 years ;

for lot 3 - must be equivalent or more than the equivalent of 120 000 USD during the last 3 years;

for lot 4 - must be equivalent or more than the equivalent of 70 000 USD during the last 3 years;

for lot 5 - must be equivalent or more than the equivalent of 140 000 USD during the last 3 years;

for lot 6 - must be equivalent or more than the equivalent of 70 000 USD during the last 3 years.

Margin of preference shall not apply.

3. Bidders may bid for one or several contracts, as further defined in the bidding document.  Bidders wishing to offer discounts in case they are awarded more than one contract will be allowed to do so, provided those discounts are included in the Letter of Bid.

4. Bidding will be conducted through the International Competitive Bidding procedures as specified in the World Bank's "Guidelines: Procurement of Goods, Works and Non-Consulting Services under IBRD Loans and IDA Credits & Grants by World Bank Borrowers"   published in January 2011 and revised in July 2014  ("Procurement Guidelines"), and is open to all eligible bidders as defined in the  Procurement Guidelines. In addition, please refer to paragraphs 1.6 and 1.7 setting forth the World Bank's policy on conflict of interest

5. Interested eligible bidders may obtain further information in Healthcare department of Zakarpattia oblast state administration, contact person Ivan Hushtan, deputy Chairman of the Selection Committee or Natalya Roshko, secretary of the the Selection Committee and inspect the bidding documents during office hours from 8:00 to 17:00at the address given below office 201-203, Narodna square 4, Uzhhorod, Ukraine,88008

6. A complete set of bidding documents may be purchased by interested eligible bidders upon the submission of a written application to the address below and upon payment of a nonrefundable fee of 1250,00 UAH or 50 USD. The method of payment will be clearing settlement. Documentation can be obtained at the office of the Project Consultancy Support Unit at presence of an appropriate errand for the bidders' representatives and a copy of payment document or may be sent by courier to the address indicated in the written application submitted by the Bidder together with the copy of the payment document.

The method of payment should be a direct money transfer to the specified account:
For UAH payment: account # 37112067018406 in the General Office of the State Treasury Service of Ukraine in Zakarpattia region, MFO local banking code 812016, USREOU code 02012852.
For USD payment: account # 26000000015423 at the Uzhhorod branch of PJSC "State export-import bmk of Ukraine", MFO local banking code 322313, USREOU code 02012852.

All bank fees are charged to the bidder.

7. Bids must be delivered to the address below or before February 21, 2018, 11:00 a.m.. Electronic bidding will not be permitted. Late bids will be rejected. Bids will be publicly opened in the presence of the bidders' designated representatives and anyone who choose to attend at February 21, 2018, 11:00 a.m..

8. All bids must be accompanied by a Bid Security of amount::
